Iron studies, and the way inflammation hides the answer

Ferritin measures stored iron and also rises with inflammation. That double role is the single most important thing to know about iron testing.

Iron testing involves several markers that answer different questions, and one of them behaves in a way that regularly confuses interpretation.

Ferritin is the usual measure of stored iron. It is also an acute phase protein, meaning it rises during inflammation regardless of how much iron is stored. Those two facts pull in opposite directions.

What each marker describes

Ferritin reflects the size of the body's iron stores. It is the single most useful marker for that question in the absence of inflammation.

Serum iron measures the iron circulating at the moment the sample was taken. It swings widely through the day, typically higher in the morning, and rises sharply after an iron tablet. It is rarely interpreted alone.

Total iron binding capacity, or transferrin, describes the carrying capacity available. Transferrin saturation is calculated from serum iron and that capacity, giving the proportion of the carrying capacity currently in use.

The inflammation problem

During infection, injury, chronic inflammatory conditions and some liver conditions, ferritin rises. A person with genuinely depleted iron stores and an active inflammatory process can therefore have a ferritin inside the reference range.

This is the reason a ferritin result is frequently read alongside an inflammatory marker such as C-reactive protein. A normal-looking ferritin with a raised CRP is a much weaker reassurance about iron stores than the same ferritin with a quiet CRP.

Transferrin saturation is less affected by inflammation and is often used to help resolve the question, which is part of why a full iron panel is ordered rather than ferritin alone.

Timing and supplements

An iron tablet taken in the day before the sample can raise serum iron substantially, and with it transferrin saturation. Where iron studies are being used to assess stores, this can produce a misleadingly comfortable picture.

Because serum iron follows a daily rhythm, morning samples and afternoon samples are not directly comparable. Where a repeat is being compared with an earlier result, matching the time of day makes the comparison more meaningful.

How iron studies relate to the blood count

Iron studies are usually read alongside a full blood count, because the consequences of depleted iron show up there. Mean cell volume in particular tends to fall when iron availability has been limited for some time.

The blood count changes later than the iron studies do. Stores can be substantially depleted before haemoglobin moves, which is why a normal haemoglobin does not settle the question on its own.

Why iron shortage is a question rather than an answer

Finding depleted iron stores rarely ends an investigation. Iron is lost slowly and absorbed slowly, so a shortage generally reflects either reduced intake or absorption, or ongoing loss.

This is why the finding usually prompts a conversation about diet, about conditions affecting absorption, and about sources of blood loss. In some groups the likely explanation is obvious; in others it is not, and the finding itself is the reason for looking further.

It is also why simply replacing iron without asking why it was low can leave the more important question unanswered.

Testing after treatment has started

Iron studies taken while a person is taking iron supplements answer a different question from studies taken before. Serum iron and transferrin saturation respond within hours of a dose; ferritin responds over weeks as stores rebuild.

Because of this, laboratories and clinicians commonly ask for supplements to be paused before repeat testing, and for the sample to be taken fasting in the morning. Where that is not practical, the timing is worth recording so the result can be read in context.

The full blood count recovers on its own timetable, later than the iron studies, which is why a haemoglobin that has not yet moved does not mean treatment is failing.

The other direction

Iron studies are also used where the question is excess rather than deficiency, and a persistently high transferrin saturation is the marker that usually prompts that line of enquiry.

A single raised value has many mundane explanations, including a recent supplement and a morning sample. Confirmation on a fasting morning sample without supplements is the usual next step before anything further.
